How It Works

Consentify makes the process of getting consent forms signed simple and paperless.

You can either use one of our pre-made templates or create a completely custom consent form that suits your needs.

Your clients can easily access the form by scanning a QR code on the spot. Alternatively, you can send them a link to complete the form beforehand — whichever works best for you.

Once the form is completed, your client’s signature is securely captured. Both you and your client will receive an email:

• To your client: A confirmation email with a thank-you note and a copy of the signed PDF attached.
• To your business: An email with all the important details (like allergies, conditions, etc.) and a copy of the signed PDF.

This process eliminates the need for paper, making things faster, more organized, and hassle-free.

For extra peace of mind, we also offer the option to store your signed PDFs securely in cloud storage, ensuring your documents are always safe and accessible.
